Additional Notes:

Upon reviewing the footage again, I saw that I cut out the "crotch" measurement segment.  I don't know why.  To find this, simply take the "slacks length" number and subtract the inseam measurement and that is your crotch measurement.

I also noted that on the chart page, there is no place that says to write the head measurement, but I still want it.

So once you're done, you can either scan the page that you wrote your notes on and send it to me, or email me the measurements using the numbers on the chart, i.e. 1=neck, 2=Shoulder to Bust Point, etc.
